TAFE audit urged over trainee jillaroo’s fatal fall

The TAFE Western Institute conducted an internal investigation but this ”failed to uncover or identify any failure of any workplace relations practices or procedures”.

”The investigation and subsequent report was essentially not worth the paper it was written on,” the coroner found.

She recommended the course be overhauled beyond the changes the TAFE had already made after the death and that safer equipment be provided to students.

As well, Education Minister Adrian Piccoli, TAFE, AgriFood Skills Australia and the Australian Skills Quality Authority should review the conduct of the horse-riding units, the TAFE contract tender process and the need for a horse’s history and suitability for use by students to be disclosed and assessed.

Speaking outside the court, Miss Waugh’s parents Mark and Juliana Waugh, who sought the inquest, said they were relieved the coroner had acknowledged their concerns that the TAFE had ”let our family down”. The course was suspended in July this year pending the inquest findings.
