A remote-controlled device was detonated at around 6 a.m. local time (0130 GMT) on Wednesday as a passenger car was passing through the Marjah district of the province.
The injured were later taken to a hospital in the provincial capital city of Lashkar Gah, located about 555 kilometers (344 miles) south of the Afghan capital Kabul, where they are in critical condition.
No group has claimed responsibility for the attack.
Roadside bombs and improvised explosive devices (IEDs) are by far the most lethal weapon Taliban militants use against Afghan forces, foreign troops, and civilians.
Insecurity continues to rise across Afghanistan despite the presence of some 130,000 US-led forces in the war-torn country.
